Capability for Work Questionnaire UC/ESA
Help with completing your work capability form - UC50 or ESA50.
Service Description
To include completing the form within the timescale (which is often tight), looking and advice to gather the right evidence, copying form if required. If you are uncertain where to start or finding the words a bit difficult, why not come and get some expert help at a time (or place if you are within 25 miles of Honiton) that is convenient for you? I might even be able to help you very quickly if your deadline is coming up (it is sometimes more difficult to get extensions for these forms compared to others).
Cancellation Policy
We will contact you as soon as we can to confirm your appointment. If you can't make your appointment - please cancel within 24 hours if you can, we appreciate that sometimes things happen on the day of appointment and we will allow you to make one more appointment if you cancel late. We can not guarantee that your application will be successful because this is the final decision of the Department of Work and Pensions (DWP) but we will do everything we can to support your application and give you the best chance of success. We recommend that you have a chat before claiming benefits that is free - we will give you our honest opinion if you would be entitled to disability benefits at this chat and if we do not feel that you have a good chance of success will not be able to support you further. You will need to provide additional evidence, it is highly unlikely that an application will be successful without this. Even free medical evidence from your GP in the form of a summary care record (that can be downloaded sometimes from your medical app) and a prescription slip are good sources of this. We do not recommend attaching photographs to your notes nor do we recommend asking the GP for a letter - these are often charged and not that useful. We will ask that you complete a diary to send in - we will provide you with access to one online that can be completed and printed or we can provide paper copies for you (this may be at an additional cost). We can send you these. Diaries are excellent evidence. We recommend that you keep copies of all medical letters that you are sent - you can ask your consultants to do this as they will be useful the next time you have to complete a form. These are the letters after appointments. Appointment letters are not good sources of evidence. We won't help people make claims that we do not believe to be true.
